ZANSKAR
FIELD TRIP ON 17th OF AUGUST
We started our journey towards Zanskar on 17th of August at 8:00
am and we had our breakfast at Nimo village. Around 10:30 am we
started again and after we reached Khalsi we took a rest. Before
Lamayuru we got some car problems and stuck for 5 hours, finally
we reached in Mulbek, where we spend our first night.
In the morning we went straight to Zanskar and we reached Arban
village at 6:30 pm.
On 19th we reached Padum village and spend three days over there
to treat our patients around in Karsha, Ruk Ruk, Stongde, and Zangla
and there we saw some new patients.
Name - Tsering Dolma Age 57/yrs old female
Diagnosis - ?
Main problem - she can't walk.
Past history - she had pain started in left knee in 2001, they went
to Doctor in Leh but they refer to Delhi and did the operation in
2002 from Delhi. She got replaced of her knee but was not success
and now she can't walk any more.
Treatment - we can't do a lot with her so we suggest doing by her
own home works like -chopping vegetables, splitting wool and told
the family to pickup wheelchair from us.
Plan - wheelchair.
After
that we found the patient
Name - Padma Youldon, Age 23/yrs old female
Diagnosis - cerebral palsy.
Problem - she can't walk and has lot of contracture in both lower
limbs and her father is always carrying her to help her in daily
routine life.
Main problem - Dependency.
History - she has only father who's looking after her and they are
so poor and helpless.
Treatment - the hygienic condition was so bad and that is why we
brought some clothes, tooth brush, soap etc.
Plan - wheelchair, bucket, warm clothes for winter.
On
23rd we started to walk from Raru village towards Lungnak Area.
On the way we found a child from Icher in Raru School -
Name - Norboo Tsering ge 13/yrs male
Diagnosis - Right foot deformity,
Problem - his all phalanges are inner rotated that way he walks
on little toe with lateral side.
History - we met him at Raru school and he said they went to Delhi
did operation of Achilles tendon 3yrs ago. This was paid for by
some foreigner.
Treatment - we did check for foot but dorsal extension is only 5
degree, toes extension is not possible and we can show some exercise
to him for whole foot pronation and standing on tips toes.
We
started again to walk for Dorzing village there we found one girl
from Dorzong,
Name
- Stanzen Konchok Age/23yrs old female
Diagnosis - Deaf and Dumb.
Problem - she can't speak and hear so she can't do normal work and
doesn't earn any money.
History - she has four family members, she is helping at home and
field work.
What
she can do by herself is tailoring and knitting.
Plan - sewing machine.
On 24th we started again to walk for Cha village there we found
four patients

- Tundup Lamo Age/38ysr old female
Diagnosis - club foot since birth right side?
Problem - hygiene situation can get a problem for a bad sore on
this foot.
Treatment - she should use her fingers do move and stretch the foot.
And she should massage the medial foot side with her hands.
Advice -She is to use a walking stick when she walks long distance
and she should do clean her feet and wear clean socks because of
the bad sore at the lateral foot side.
Sonam
chombel Age/37yrs old s/o Kunga Norboo - village Cha
Diagnosis - ?
Problem - eye problem
Plan - sun glasses.
Stanzen
Dolma Age/25yrs old
Diagnosis - scoliosis left convex thoracic vertebrae.
Problem - Breathing problem.
Treatment - side position left - stretch right side trunk left arm
over head, extended leg, breath deeply.
- four foot position
- Cat -extension and flexion in the trunk.
- Stretching right trunk look to the left foot.
- Push up and down with nose almost to ground.
- Strengthen the trunk muscles.

Palchok Age/18yrs old male
Diagnosis - cerebral palsy with hemiplegia right side.
Problem - He will get contracture when he doesn't wear good shoes
with growing.
Treatment - four foot position
- Dorsal extension passively and bring more weight on the straight
arms.
- prevent contracture in the wrist.
- Foot - standing and walking with tiptoes right foot but right
leg is shorter.Dorsalextension also actively possible.
Plan - we will bring him sport shoes with rising 1cm right side
to prevent contracture in the ankle.
- Present history - He told us that his doing field work and he
helps at home.
Thupstan
Stanzen Age/48ysr old male
Diagnosis - knee Deformity,
Problem - He has pain and the skin situation is bad.
Flexion contracture left side and this influences the right side.
Treatment - painful in the knee forefoot, especially while walking.
Much pressure on the phalanges of the left foot.
We gave crutches to him, walking looks much better,
Trunk is straighter and hips more extended, less pain.
Advice - We told him put some blanket under the bent knee in lying
position.
History - He's living alone at home and doing his field work.
Lobzang
Eshay Age/14yrs old
Diagnosis - Foot Deformity left side after ankle was broken 11yrs
ago.
Problem - Contracture of the foot and influence in the whole body.
Advice - We advice to him to move toes and whenever his walk he
should walk with the walking stick.

yangskit Age/49yrs female
Diagnosis - scoliosis left convex thoracic vertebrae rotated to
right side, ribs are come out to left side.
Problem - Breathing problem.
Treatment - four foot position,
- cat - extension and flexion vertebrae - to get more mobility,
- Rotation in the vertebrae left one arm up and looks in the hand
- to get more mobility.
- Push up and nose must be touching the ground - to strength the
muscle.
History - she has seven children and they are all grown up and older.
She can do most of the work but, can't carry heavy things.

Konchok Age/16yrs old male
Diagnosis - Deaf and Dumb,
Problem - He couldn't go to school, he has no education,
History - He has no physical problem, he can understand everything
and work with his family on the field.
Plan - we can look for some special school for him.

chonsom Age/15yrsold female
Diagnosis - slow mind and balance problem,
Problem - Balance problem,
Treatment - we did one leg standing, but it's difficult for her
- only 4second. Both legs are the same.
- Walking on a line was possible, but much movement in the trunk
- Balance was decrease and we tried jumping.
History - we found one patient from Thangsoc School but we don't
know much more about her family background.

Diagnosis - Hearing problem.
History - she's living alone and no family members.
Plan - Hearing aids, we try to do fine some hearing aid.

Diagnosis? Problem - eye problem.
Plan - Next we bring sun glasses for her.
History - we found some patient from Kargak School.

Diagnosis - Deaf and Dumb.
Problem - Hearing and speech but she can understand, (No physical
problem)
History - She is able to knit (like socks and sweater) and she helps
at home.

Diagnosis -? Mentally problem.
Problem - she can't speak a lot.

Diagnosis - Hypotension.
Problem - she is very afraid and always stayed with mother
Advice - we suggest the mom not to carrying her to much and let
her walk by herself.
We left one playing ball for her.

Diagnosis - ? Hearing but he uses hearing aid and his hearing now.

Problem - He is not going to school and stayed at home, he is helping
at home.
Plan - We told the parents to bring him to Leh so that he can learn
to do some work like carpenter and can earn by him self.

Galsen Age/40yrs old male
Diagnosis - scoliosis left convex in chest, kyphosis.
- Hypo tones and lattisimus under the shoulder,
Problem - Breathing problem.
Treatment - fourfoot position -
- Cat- extension and flexion in vertebrae.
- Rotation in the vertebrae left one arm up and looks in the hand.
- Prone position and push up and nose must be touching the ground.
History - most of the time his staying at home and helping in kitchen
work.

Diagnosis - cerebral palsy (otisum?)
Problem - she can't speak and no coordination.
History - we met her on the field with mom, we don't know much more
about family background.
On
the way back we did survey in Kargil.

Diagnosis - He has lot of contracture in lower limb.
Problem - He is totally dependent and in his daily routine life.
Advice - we advice his father to move the legs of his son and change
his position frequently, for lying in same position is bad for him.
Plan - Next time we bring to wheelchair for him.

Diagnosis - Tuberculosis in left hip joint.
Problem - walking.
Advice - we advice to him whenever his walk he should walk with
the walking stick.

Diagnosis - Balance and mentally problem (mentally little bit)
History - He has two brother but all three have problems.
Plan - Next time we bring shoes for him.

Diagnosis - right leg is shorter for 4cm and rotated outward.
Problem - when she is walking a lot.
History - she has three children and she can do most of the work
at field and home.
Plan - Next time we bring shoes for her and we see to it, that the
right shoe gets a rising.

Diagnosis - Mentally and physically problem.
Problem - Balance and coordination.
History - we met her on the road that way we don't know much more
about family background.
We gave one toy like wing chin (voice once).
Gulam Ramad Age/10yrs old male
Diagnosis -? Himiplegia left side.
Problem - Balance little bit.
History - we met him on the way with grand father. He is going to
school and we don't know much more about his background.
We enjoyed this field trip and much more enjoyed in Lunang area
because we did walk 6days in this valley and its so beautiful villagers
in there.
Finally we reached in leh on 1september at 12: pm.
